Summary

The co-administration of clarithromycin with darunavir and ritonavir shows a decrease in darunavir AUC (13%) and an increase in clarithromycin AUC (57%). Dosage adjustments of clarithromycin are recommended for those patients with renal impairment with concomitant use of darunavir plus ritonavir: CrCl 30-60mL/min decrease clarithromycin dosage 50% and for CrCl <30mL/min decrease clarithromycin dosage 75%; dose adjustments for this agent is not required for those patients with normal renal function; however clinical monitoring is warranted.
